Team Leader — Community Health, Primary Health Care

Nova Scotia Health is hiring a Team Leader for Community Health, Primary Health Care, based in Halifax with the Chebucto Community Health Team, Central Zone.

This permanent full-time leadership role oversees the delivery of community-based primary care programs and works with teams and partners to improve local wellness outcomes. Key responsibilities include program evaluation, quality improvement, and supporting health promotion and chronic disease prevention initiatives.
