Reporting to the Public Health Early Years Manager, the Community Home Visitor (CHV) works in collaboration with the Early Years Consultant and others as appropriate to ensure Early Years programs and services are planned and implemented to address local population health needs.
The CHV works with at risk families and vulnerable populations during the prenatal, postpartum, and early childhood period. The CHV implements the Healthy Beginnings Enhanced Home Visiting program including strengths based curriculum for at risk families. The CHV is part of a collaborative multidisciplinary team approach and works with the Early Years Team to ensure quality case management. The CHV shares a core set of public health attitudes and values, applies public health core competencies to all the work and is accountable to provide competent, safe and ethical professional practice. The CHV ensures that the principles and practices that guide the Nova Scotia Health such as the vision, mission, values, strategic directions, ethics, safety, quality, partnerships and inter-professional collaboration are integrated within the work that they do and through the messages delivered.
