Case Manager, Place Based
This position is designated for individuals who identify as Indigenous.
The Case Manager provides ongoing support to people exiting homelessness through the Housing First program, helping them secure permanent housing and maintain stability.
Responsibilities include developing strength-based recovery plans grounded in Housing First and harm reduction principles; managing a caseload of 15 to 30 individuals; assisting with housing searches, viewings and applications; providing crisis response and connecting participants to resources; and maintaining documentation and communication with agencies, landlords and community partners.
Qualifications: post-secondary education in human services, psychology or social work, or an equivalent combination of education and work experience; two years of experience working with at-risk participants; and strong communication, documentation and interpersonal skills. Knowledge of Housing First and harm reduction principles is preferred.
Full-time in Halifax. $29.42 per hour. No closing date is stated; the posting is open until filled.
